Introduction: Purpose is to unveil the uncommon presentation of retinoblastoma presented in an ophthalmology clinic.

Methodology: A retrospective descriptive study was done in which all the retinoblastoma suspects presented to Eye Clinic in a tertiary hospital, Lahore during 2018- 2023 were included in the study.

Results: A total of 415 children presented to retinoblastoma services as retinoblastoma suspect, and 370 were diagnosed as retinoblastoma based on clinical presentation, MRI & B-Scan. A considerable number of the diagnosed children were in the older age group with unusual presenting complaints other than common presentation as leukocoria or strabismus.

Conclusion: It is imperative to consider all children presented with ambiguous history of trauma, or red eye, or found to have Coat’s disease like exudation on fundus examination, as retinoblastoma suspect and investigate them thoroughly.
