Introduction: The cancer treatment needs precautions and drug therapy for a long duration. Patient understanding about the illness plays a vital role in management of oncology patients. The pharmacist has major responsibility in counseling the patients with chronic illness. The counseling pharmacist should possess adequate knowledge and should be an effective communicator, making use of the verbal and non-verbal communication skills.

An error reporting model affords pharmacists the opportunity to make valuable contributions, including improved outcomes and positive economic impacts such as cost savings and elimination of dispensing errors before going to patients. Health-system pharmacists have a positive impact in improving healthcare through programs that improve efficiency, safety, and clinical outcomes of medication use.

Methodology: The data used in the study was collected at the outpatient counseling department at the pharmacy of Children Cancer Hospital Egypt.

It is a descriptive study conducted over a period of six months, i.e. Feb to July 2023. A total of 54000 patients dispensed in the outpatient department in course of study. An average of 300 counseling sessions were being done daily. An average of 3 interventions per day and a total of 410 interventions in six months were reported from Feb to July 2023, Interventions data collected for six months and assessed for improving patient education and reducing medication errors.

The data used in the study was collected at the outpatient counseling department of pharmacy at Cairo Egypt.

Conclusion: This study concludes that the involvement of clinical pharmacists in patient counseling at outpatient pharmacy has a positive impact in creating awareness about the disease and medicine usage for better patient management. This involvement of clinical pharmacists reduces medication errors, prescribing and dispensing errors that lead to better therapeutic outcomes.
